NHS Agenda For Change 2026/27: Pay Scales & Bands
NHS AfC pay bands 2026/27 at a glance (England, effective 1 April 2026, 3.3% uplift):

NHS Agenda for Change Pay Bands 2026/27 — Full Pay Scales (England)
All NHS Agenda for Change (AfC) staff in England, Wales, and Northern Ireland received a confirmed 3.3% pay rise from 1 April 2026 — the first on-time April payment in six years. Scotland secured a better deal at 3.75% under a separate two-year agreement with the Scottish Government. These are not the same figures — if you work in Scotland, your band pays slightly more than the England equivalent.

AfC covers over 1.3 million NHS staff — nurses, midwives, paramedics, physiotherapists, radiographers, porters, and administrative staff — slotted into Bands 2 through 9 based on job weight and responsibility assessed through the NHS Job Evaluation Scheme. Doctors, dentists, and very senior managers sit outside AfC entirely; their pay is reviewed separately by the DDRB. The NHS pay rise 2026/27 guide covers the full award background and what it means for take-home pay.

NHS Pay Bands 2026/27 — Scotland (3.75%)
Scotland secured a separate two-year deal at 3.75% — 0.45 percentage points higher than England. This applies to NHS Scotland staff under the equivalent AfC framework, implemented via the Management Steering Group (MSG). The full devolved figures and how Scottish income tax affects take-home are covered in the NHS Scotland pay rise 2026/27 guide. Scottish Band 5 nurses, for example, start slightly above the £32,073 England entry point once the 3.75% uplift is applied — and Scottish income tax rates mean take-home calculations differ significantly from England at the same gross salary.

NHS Pay Bands 2026/27 — Wales and Northern Ireland
Wales and Northern Ireland both received the same 3.3% uplift as England, negotiated through their respective governments. The Welsh Government and the Department of Health Northern Ireland publish their own pay circulars. Spine point figures align closely with England but may have minor regional differences. If you work in Wales or Northern Ireland, check your health department’s pay circular for the confirmed figures applicable to your employer.
High Cost Area Supplement (London Weighting) 2026/27
If you work in or near London, you receive a High Cost Area Supplement (HCAS) on top of your basic band salary. From 1 April 2026:
| Zone | Supplement | Minimum | Maximum |
|---|---|---|---|
| Inner London | 20% of basic | £5,794 | £8,746 |
| Outer London | 15% of basic | £4,870 | £6,137 |
| London Fringe | 5% of basic | £1,346 | £2,270 |
A Band 5 nurse at entry (£32,073) working in Inner London receives 20% = £6,414.60 HCAS, bringing total salary to approximately £38,487 — paid alongside basic pay each month. The HCAS is subject to the minimum and maximum cap for each zone.

How Agenda for Change Pay Progression Works
Pay progression within a band is time-served at that band, not annual performance ratings — but since April 2019 it is conditional on demonstrating the required knowledge, skills, and competencies at your appraisal. Your employer must carry out an appraisal no later than three months before your progression date. If deferred, your manager must formally notify you in writing with an action plan.
Progression structure by band group:
Bands 2, 3, and 4: Two steps. Entry to top after 2 years (Bands 2 and 3) or 3 years (Band 4).
Bands 5, 6, and 7: Three steps — entry, intermediate, top. Intermediate after 2 years, top after a further 2–3 years.
Bands 8a to 9: Three steps. Entry to intermediate after 2 years, intermediate to top after a further 3 years. There is no automatic advancement between 8a, 8b, 8c, and 8d — each is a separate job level requiring a new appointment.
For a full breakdown of how appraisals, step dates, and deferral rules work, see the NHS pay progression appraisals and increments guide. If you are aiming for a higher band through promotion, the how to move up an NHS pay band guide covers the process end to end.
What Is My Take-Home Pay After Deductions?
Gross salary is not what lands in your bank account. Deductions include income tax, National Insurance, and NHS pension contributions — which are tiered by earnings. The NHS take-home pay calculator gives you a personalised net figure for your band, step, nation, and tax code.
Approximate monthly take-home for Band 5 in England 2026/27 (standard tax code, 9.8% pension tier):
| Pay Step | Gross Annual | Approx. Monthly Take-Home |
|---|---|---|
| Entry (£32,073) | £32,073 | ~£2,280 |
| Intermediate (£34,592) | £34,592 | ~£2,440 |
| Top (£39,043) | £39,043 | ~£2,700 |
For part-time staff, salary and all benefits are pro-rated to contracted hours. A Band 5 nurse working 22.5 hours per week (0.6 WTE) earns 60% of £32,073 = £19,243.80 per year. Sick pay, annual leave, and pension accrue on the same pro-rata basis. Model this using the NHS part-time take-home pay calculator.
Annual Leave and Key Benefits Under AfC 2026/27
Your band and salary are only part of the picture. AfC also sets your entitlement to:
Annual leave — based on length of NHS service, not band. New starters receive 27 days plus 8 public holidays; after 5 years, 29 days; after 10 years, 33 days. Calculate your exact entitlement with the NHS annual leave calculator.
NHS sick pay — significantly more generous than statutory minimum. After 5 years’ service: 6 months full pay then 6 months half pay within any rolling 12-month period. Entitlement builds from year 1. Previous NHS service with any employer counts as long as the break was under 12 months. Use the NHS sick pay calculator to see your current entitlement.
NHS pension — automatically enrolled, defined benefit, with employer contributions of 23.7%. Employee contributions run from 5% to 14.5% depending on earnings tier. The NHS pension calculator shows projected retirement income and how opting out affects your take-home now versus later.
Maternity pay — 8 weeks full pay, then 18 weeks half pay plus Statutory Maternity Pay, then 13 weeks SMP only. Requires 12 months’ continuous service at the 11th week before expected childbirth. Model your entitlement with the NHS maternity pay calculator.
Unsocial hours and overtime — evenings, nights, weekends, and bank holidays attract enhanced rates. Overtime beyond contracted hours should be paid at enhanced rates or taken as TOIL. Full rates and calculations covered in the NHS overtime and unsocial hours guide.
How Your Pay Band Is Decided: NHS Job Evaluation
Every NHS role is assessed against the AfC Job Evaluation Scheme — scored on knowledge and skills required, complexity, degree of responsibility, physical and mental demands, and working conditions. The total score determines the band. Where a national job profile exists, the role is matched to it; otherwise it is evaluated locally against the same criteria.
If you believe your role is incorrectly banded, you have the right to request a review through your employer’s appeals process. Around 5% of staff have historically appealed initial bandings. Clear evidence of actual job duties compared to the national profile is essential for a successful regrading.
What About Band 1?
Band 1 closed to new entrants from 1 December 2018 as part of the 2018 pay deal reforms. Any remaining staff still on Band 1 contracts are paid at the Band 2 rate of £25,272. If you joined the NHS after December 2018, you will not be on Band 1.
NHS Pay Circular 2026 — Key Dates
The 2026/27 AfC pay rates were confirmed by NHS Employers in February 2026, took effect 1 April 2026, and were implemented in the NHS Terms and Conditions of Service Handbook accordingly. Scotland’s 3.75% settlement was agreed separately under a two-year deal with the Scottish Government. The Pay Circular (M&D) is the definitive legal reference document — it is published by NHS Employers and updated when new awards come into effect.

Do Scotland’s NHS pay scales differ from England?
Yes. Scotland received 3.75% for 2026/27 under a separate agreement, meaning all band salaries are slightly higher than their England equivalents. Scottish income tax bands also differ from England, affecting take-home pay at the same gross salary. The NHS Scotland pay rise guide covers all devolved figures.

Can my employer pay me outside AfC scales?
In most cases, no. Some trusts offer recruitment and retention premia (RRP) for hard-to-fill posts — these are additions to, not replacements for, the AfC pay point. The NHS RRP guide explains how these work and who qualifies.
